• Copy of your current insurance card
• Current remaining plan deductible amount
• Current co-payment and/or co-insurance amounts

Enter your name and insurance or payment information.

If you have insurance:
Some insurance providers are directly linked to My Cost and will automatically enter your deductible, co-payment and co-insurance amounts. If your insurance company is not directly linked, you will enter those amounts yourself. Simply follow the My Cost screens for instructions.

If you do not have insurance:
Answer a series of questions to see if you’re eligible for financial assistance.

Select the Alegent Health procedure you need.

Get an estimate for your procedure and find out your out-of-pocket cost based on the insurance information you provided. Or if you choose not to use insurance, find out your estimated self-pay price.
